List of Contributors

This National Survey on Drug Use and Health (NSDUH) report was prepared by the Division of Population Surveys, Office of Applied Studies (OAS), Substance Abuse and Mental Health Services Administration (SAMHSA), U.S. Department of Health and Human Services (HHS), and by RTI International (a trade name of Research Triangle Institute), Research Triangle Park, North Carolina. Work by RTI was performed under Contract No. 283-2004-00022.

At SAMHSA, Arthur Hughes was responsible for the content, analysis, and writing of the report, and Joseph C. Gfroerer reviewed and provided editorial suggestions. At RTI, Neeraja Sathe and Kathy Spagnola were responsible for the writing of the report, and Ralph Folsom and Akhil Vaish were responsible for the overall methodology and estimation for the model-based Bayes estimates and prediction intervals.

The following staff were responsible for generating the estimates and providing other support and analysis: Akhil Vaish, Neeraja Sathe, Kathy Spagnola, and Brenda Porter. Ms. Spagnola provided oversight for production of the report. Mary Ellen Marsden reviewed the report, Richard S.
