This report was prepared by the Office of Applied Studies (OAS), Substance Abuse and Mental Health Services Administration (SAMHSA), and by RTI International (a trade name of Research Triangle Institute), Research Triangle Park, North Carolina. Work by RTI was performed under Contract No. 283-2004-00022. At SAMHSA, Arthur Hughes was responsible for the content, analysis, and writing of the report, and Joseph C. Gfroerer reviewed and provided editorial suggestions. At RTI, Neeraja Sathe and Kathy Spagnola were responsible for the writing of the report, and Ralph E. Folsom was responsible for the overall methodology and estimation for the model-based Bayes estimates and prediction intervals.
